The Kenya Traveller’s Health Surveillance Form is a digital registration for travellers planning to visit Kenya.
The Health Form is part of the Kenya’s entry requirements implemented to ensure the safety of the general public.
All travellers are required to complete the Health Surveillance Form for Kenya separately within 72 hours of the planned arrival in Kenya.
The form must be filled out with the basic personal and contact details of the incoming visitors, the details of their planned trip to Kenya, as well as health-related information.
Completed forms are processed by the Kenyan Government, which then issues PDF confirmations with unique QR codes for every traveller who has submitted their form online.

Kenya Traveller’s Health Surveillance Form is obligatory for every person planning a trip to Kenya during the Covid-19 pandemic.
Travellers must complete the form separately and therefore have separate confirmations of a completed form upon entering Kenya.
Please note! Underage travellers’ data cannot be added to their parents or legal guardians’ forms.
Parents or legal guardians should take care of completing a separate form for underage travellers, making sure all the data provided is correct and up-to-date.
